Background & objectives: Urinary tract infections (UTIs) caused by extended-spectrum beta lactamase (ESBL)-producing bacteria، Metallo-Beta-Lactamase (MBL) have become a growing problem worldwide. The aim of this study was to Uropathogenic Bacterial and Antibiotic Resistance Pattern among patients Refereeing to Imam Khomaini Hospital, Ilam, Iran
Methods: In this cross - sectional study of patients with urinary tract infection in Imam Khomeini Hospital in Ilam were evaluated over a period of 9 months. All isolates were identified by routine biochemical methods and antimicrobial susceptibility testing carried out by Kirby-Bauer method. Phenotypic confirmation test to detect ESBL, MBL, MRSA, VRSA and VRE antibiotic disk diffusion method was performed by disc. The results were interpreted according to the CLSI standard. Identification of genes SHV, CTX-M, TEM, VanA, mecA, bla VIM and bla IMP PCR was performed using the method.
Results: 114 isolates were examined by 61.4% Escherichia-coli, 7% Citrobacter freundii, 7% Pseudomonas aeroginosa, 8% Enterobacterccloacae, 8% of Klebsiella poneumoniue, 4.1%Proteus mirabilis, 0.9% Acintobacter baumannii, 0.9% of Staphylococcus aureus, 0.9% Enterococcus faecalis and 1.8% diphtheroids were separated.
Conclusion: Most isolates were resistant to the antibiotic cotrimoxazole (55%), cefotaxime (49%), ceftriaxone (41%) showed. 46 isolates (49.5%) ESBL were considered. Escherichia-coli 63%, 8% Citrobacter freundii , Pseudomonas aeroginosa 6%, 6%, Enterobacter cloacae, Proteus mirabilis 6%, 6% and 2% of Klebsiella pneumoniae were Acintobacter baumannii . In total, only 6.4% of the isolates were considered as phenotype MBL. The genotyping PCR method was shown that genes VIM1, VIM2 and IMP were not mentioned in any of the isolates. In between isolates of Gram-positive, none genotyped MRSA, VRSA and VRE was isolated. Only one (100%) Staphylococcus aureus isolates, for phenotypic VRSA was considered. The results of this study, as well as Gram-negative bacteria Enterobacteriaceae family, are the main factors that cause urinary tract infections.The results of this study show that the ESBLs produced in bacteria isolated from patients with urinary tract infection, which is up almost 49.5% of the isolates were ESBL and 6.4% MBL. Since, according to the study in Ilam high prevalence of ESBL-producing bacteria in the urinary tract infections and the prevalence is rising MBL. It is recommended that programs be seriously considered antibiotic use and prescribing of antibiotics is organized. As well as more studies are done on the MBL.
