Diarrhea is still a major threat to the young population in low- and middle-income countries (1) and is a leading cause of death in children (2).
According to the WHO, after HIV, the second leading cause of death in children under 5 is diarrhea (1). In 2015, diarrhea accounted for 8% of all under-5 deaths worldwide (3), and is also a problem not only in health but also in the economy due to the frequent hospitalization of children (4). ).
Laboratory diagnosis of gastroenteritis pathogens is mainly based on microscopy, culture and immunoassay (5, 6), but these methods are time consuming or sometimes insensitive (7) and the delivery time of these tests also varies, for example in Campylobacter species and Or Salmonella which can last for several days (5). In developing countries, the diagnosis of diarrheal infections generally relies on clinical signs, which usually do not lead to accurate identification of the pathogen causing the disease, and in the absence of specialized tests, infections are managed using an experimental antibiotic regimen. This is often associated with overuse of antibiotics and the spread of bacterial resistance (8), so the need for a rapid and reliable diagnostic method is felt.
Molecular methods have high sensitivity and specificity and with the help of it, bacterial agents can be identified simultaneously and specifically (9). Multiplex PCR experiments in recent years have paved the way for the diagnosis of gastrointestinal diseases (10).
 Multiplex PCR is a fast and accurate method for detecting fecal pathogens (11). This method has shown high sensitivity compared to conventional methods for detecting most pathogens (10). Multiplex PCR is better because of its many advantages, including: simultaneous detection of a wide range of pathogens, faster time, lower costs and higher efficiency, and this leads to the use of this diagnostic tool in epidemiological studies of diarrheal infections (12). ).
Rapid and simultaneous amplification of several genes in one reaction is one of the goals pursued in Multiplex PCR. One of the features of this test, in addition to its speed and cost-effectiveness, is to minimize the possibility of contamination in the studied samples (13).
Having information about the prevalence of common diarrhea pathogens can be effective in controlling and properly managing the disease among Iranian infants and children. Therefore, due to the lack of sufficient information about the role of common gastrointestinal pathogens in pediatric diarrhea in Ilam, as well as the lack of appropriate diagnostic methods, gastrointestinal diseases of children are always considered as major health problems. On the other hand, most physicians rely on clinical symptoms. Therefore, finding a suitable method and diagnosis that is also fast, can help doctors in treating children for gastrointestinal infections in Ilam.
Therefore, by conducting this study, the prevalence of gastrointestinal bacteria in children in Ilam has been determined and a new perspective in helping treatment will be provided to physicians.
