بررسی شیوع انتروپاتوژن های شایع باکتریایی در کودکان مبتلا به اسهال در شهر ایلام، به روش Multiplex PCR

<td style="height:395px;">بیماری اسهال هنوز هم یک تهدید بزرگ برای جمعیت جوان در کشورهای کم درآمد و یا با درآمد متوسط بوده(1) و از دلایل اصلی مرگ و میر در کودکان است(2) .<br>
طبق گزارش سازمان <span dir="LTR">WHO</span> بعد از ویروس <span dir="LTR">HIV</span> دومین علت مرگ در کودکان زیر 5 سال، اسهال می باشد(1). در سال 2015 بیماری اسهال 8<span dir="LTR">%</span> از کل مرگ و میرهای کودکان زیر 5 سال در سراسر جهان را تشکیل داده است(3) ، همچنین بدلیل بستری شدن مکرر کودکان، باعث یک مشکل نه تنها در سلامت، بلکه در اقتصاد نیز می باشد(4).<br>
تشخیص آزمایشگاهی پاتوژن های گاستروانتریت عمدتا برپایه میکروسکوپی ،کشت و ایمونواسی است(5 و 6) ، اما این روش ها وقت گیر می باشند و یا گاها حساسیتی ندارند(7) و زمان تحویل این آزمایش ها نیز متغییر است مثلا در گونه های <em>کمپیلوباکتر</em> و یا <em>سالمونلا</em> که می تواند تا چندین روز باشد(5) . در کشورهای در حال توسعه نیز تشخیص عفونت های اسهالی بطور کلی، متکی بر علائم بالینی است که این معمولا منجر به شناسایی دقیق پاتوژن عامل بیماری نمی شود و از طرفی در غیاب آزمایش های تخصصی، عفونت ها با استفاده از رژیم آنتی بیوتیک تجربی مدیریت می شوند که این اغلب با استفاده بیش از حد آنتی بیوتیک و گسترش مقاومت باکتریایی همراه است(8) پس نیاز به روش تشخیصی سریع و قابل اعتمادی احساس می شود.<br>
روش های مولکولی دارای حساسیت و اختصاصیت بالا بوده و با کمک آن می توان عوامل باکتریایی را بطور همزمان و اختصاصی شناسایی کرد(9) . آزمایشات مبتنی بر واکنش چندگانه زنجیره ای پلیمراز (<span dir="LTR">Multiplex PCR</span> ) در سالهای اخیر راه خود را در زمینه ی تشخیص بیماری های گوارشی باز کرده اند(10 ).<br>
<span dir="LTR">Multiplex PCR</span> روشی سریع و دقیق برای تشخیص پاتوژن های مدفوعی است(11). این روش حساسیت بالایی نسبت به روش های معمولی برای تشخیص اکثر پاتوژن ها نشان داده است(10). <span dir="LTR">Multiplex PCR</span> بدلیل مزایای بسیار از جمله: تشخیص همزمان طیف گسترده ای از پاتوژن ها، زمان سریع تر، کاهش هزینه ها و بازده بالاتر، بهتر می باشد و همین، موجب استفاده از این ابزار تشخیصی در مطالعات اپیدمیولوژیک عفونت های اسهالی می شود(12).<br>
تکثیر سریع و همزمان چند ژن در یک واکنش یکی از اهدافی است که در <span dir="LTR">Multiplex PCR</span> دنبال می شود. از ویژگی های این تست علاوه بر سریع و مقرون به صرفه بودن آن، به حداقل رساندن احتمال آلودگی در نمونه های مورد بررسی می باشد(13).<span dir="LTR"></span><br>
داشتن اطلاعاتی در مورد میزان شیوع پاتوژن های شایع اسهال می تواند در کنترل و مدیریت صحیح بیماری در میان نوزادان و کودکان ایرانی موثر باشد. لذا باتوجه به عدم اطلاعات کافی درمورد نقش پاتوژن های شایع دستگاه گوارش در اسهال کودکان در شهر ایلام، و همچنین عدم روش های تشخیصی مناسب، همواره بیماری های گوارشی کودکان از مشکلات عمده بهداشتی بحساب می آیند. از طرفی اکثر پزشکان، درمان را با اتکا به علایم بالینی انجام می دهند. بنابراین یافتن روشی مناسب و تشخیصی که نیز سریع باشد، می تواند پزشکان را در امر درمان کودکان برای عفونت های گوارشی در شهر ایلام کمک کند.<br>
از این رو با انجام این مطالعه، وضعیت شیوع باکتری های دستگاه گوارش در کودکان شهر ایلام مشخص شده و چشم انداز جدیدی در کمک به درمان در اختیار پزشکان قرار خواهد گرفت.<br>

Diarrhea is still a major threat to the young population in low- and middle-income countries (1) and is a leading cause of death in children (2).<br>
According to the WHO, after HIV, the second leading cause of death in children under 5 is diarrhea (1). In 2015, diarrhea accounted for 8% of all under-5 deaths worldwide (3), and is also a problem not only in health but also in the economy due to the frequent hospitalization of children (4). ).<br>
Laboratory diagnosis of gastroenteritis pathogens is mainly based on microscopy, culture and immunoassay (5, 6), but these methods are time consuming or sometimes insensitive (7) and the delivery time of these tests also varies, for example in Campylobacter species and Or Salmonella which can last for several days (5). In developing countries, the diagnosis of diarrheal infections generally relies on clinical signs, which usually do not lead to accurate identification of the pathogen causing the disease, and in the absence of specialized tests, infections are managed using an experimental antibiotic regimen. This is often associated with overuse of antibiotics and the spread of bacterial resistance (8), so the need for a rapid and reliable diagnostic method is felt.<br>
Molecular methods have high sensitivity and specificity and with the help of it, bacterial agents can be identified simultaneously and specifically (9). Multiplex PCR experiments in recent years have paved the way for the diagnosis of gastrointestinal diseases (10).<br>
Multiplex PCR is a fast and accurate method for detecting fecal pathogens (11). This method has shown high sensitivity compared to conventional methods for detecting most pathogens (10). Multiplex PCR is better because of its many advantages, including: simultaneous detection of a wide range of pathogens, faster time, lower costs and higher efficiency, and this leads to the use of this diagnostic tool in epidemiological studies of diarrheal infections (12). ).<br>
Rapid and simultaneous amplification of several genes in one reaction is one of the goals pursued in Multiplex PCR. One of the features of this test, in addition to its speed and cost-effectiveness, is to minimize the possibility of contamination in the studied samples (13).<br>
Having information about the prevalence of common diarrhea pathogens can be effective in controlling and properly managing the disease among Iranian infants and children. Therefore, due to the lack of sufficient information about the role of common gastrointestinal pathogens in pediatric diarrhea in Ilam, as well as the lack of appropriate diagnostic methods, gastrointestinal diseases of children are always considered as major health problems. On the other hand, most physicians rely on clinical symptoms. Therefore, finding a suitable method and diagnosis that is also fast, can help doctors in treating children for gastrointestinal infections in Ilam.<br>
Therefore, by conducting this study, the prevalence of gastrointestinal bacteria in children in Ilam has been determined and a new perspective in helping treatment will be provided to physicians.
