The ASCII clinical trial is a method of assessment in which a student goes through different stations and is evaluated on a variety of topics. Each station includes a clinical program in which it interviews patients or includes an issue. It is the clinic where the volunteer is asked to interpret and solve it.
Each candidate's performance is marked by an evaluator in a structured checklist dedicated to that station. (1)
This test, like other assessment methods, has advantages and limitations; this method measures a wide range of skills. Measures cognitive goals, skills, and to some extent attitudes.
Its scoring is accurate and its feedback is instantaneous, but its disadvantages include high cost, time consuming and creating anxiety in the student. (2)
Studies show that ASCII test anxiety is largely related to test time, complexity, and techniques, as well as confusion about how to test. (3)
Due to the fact that one of the points of student anxiety is the student's unfamiliarity with how to hold this test, so we find that more solutions should be addressed in which the student is informed about the process of this test.
In a study conducted in Egypt to investigate the effect of ASCII test on nursing students, the student's previous experience of taking the ASCII test in the previous year affected the outcome of the study and caused two thirds of students to say that this method is appropriate. Has been and compared to other tests has created the least stress in them. (4) In a study conducted by Bagheri et al., This test was considered stressful, but this result was affected by not informing the students, the interval of one semester between the training of the course unit and the time of holding the test in Aski style. (5)
After these studies, we find that the more familiar students are with this test, the better their performance will be.
Evidence shows that in the clinical evaluation system of Iranian nursing and midwifery students, ASCII is a valid and reliable test and in order for students to perform better during the test, more detailed planning is needed before the test. (6)
A workshop is an organized set that provides opportunities for individuals to learn through thought, action, and discussion.
The term workshop ﺑﻪ ﺑﻪ ﺑﻪ ﺑﻪ ﺑﻪ. ﺑﻪ ﺑﻪ ﺑﻪ ﺑﻪ
One of the workshops that will help people achieve their goals and achieve their goals. (7)
All of this reduces students' anxiety and greater preparation for the ASCII test.

Introduction:
Functional abdominal pain is a continuous or nearly continuous pain. There is no
relationship between physiological events and pain. The pain will lose some os daily
functions and is not feigned. There isn't sufficient symptoms to meet criteria for
another functional gastrointestinal disorder. This criteria must be fulfilled for the last 3
months with symptom onset at least 6 months prior to diagnosis. The pain is very
common in children and although benign in nature is associated with anxiety, school
absenteeism, frequent clinic visits, unnecessary testing and significant economic
burden.
Material and methods:
This study was conducted in the form of double blind clinical trial on a statistical
population of 104 children aged 5-15 years old who were suffering from functional
abdominal pain with the above criteria. In this study, after blood, urine and stool
sample analysis and performing an abdominopelvic sonography to rule out organic
disorders, the patients were divided into two groups: One group went under treatment
with saccharomyces boulardii (250mg BDS) and the other group started the process
using placebo. The patients took drug or placebo for three weeks and were also
followed for three weeks without medication. During this six week period, the severity
of the patient’s pain was assessed by trained mothers based on oucher pain scale.
Eventually the data was analyzed by statistical software SPSS.
Results:
57 of the patients were girls and the other 47 were boys. 55% of the patients were 5-10
years old and 45% were 10-15 years old. Using variance analysis, we compared the
pain score in treatment and control groups and found out that the drug effects show no
significant difference in various age (p-value: 0.928) or sex (p-value: 0.437) groups.
Comparing the control and treatment groups, Treatment with saccharomyces boulardii
was proved to be effective (p-value: 0.00); and the least amount of pain and the
highest difference between the two groups was seen in the fifth week.
Conclusion:
Considering the effectiveness of saccharomyces boulardii in treating children’s
functional abdominal pain, this probiotic can be used to cure this pain, which will
result in reduced school absenteeism, anxiety, frequent clinical visits and economic
burden.

Background

Thyroid dysfunction during pregnancy is associated with adverse outcomes for both mother and fetus. The present meta-analysis was conducted to evaluate thyroid dysfunction in Iranian pregnant women.

Methods

We registered this review at PROSPERO (registration number: CRD42020166655). The research steps in this systematic review and meta-analysis were performed according to the MOOSE protocol, and finally, reports were provided based on the PRISMA guidelines. The literature search was performed in October 2019 using the international online databases, including Web of Science, Ovid, Science Direct, Scopus, EMBASE, PubMed/Medline, Cochrane Library, EBSCO, CINAHL, Google Scholar as well as national databases were reviewed. Data were extracted after applying the inclusion and exclusion criteria and qualitative evaluation of the studies. I2 index and Q test were used to assess differences in studies. All analyses were performed using Comprehensive Meta-Analysis Software. P-value less than 0.05 was considered statistically significant. We identified 1261 potential articles from the databases, and 426 articles remained after removing the duplicate and unrelated studies. After evaluating the full text, 52 articles were removed.

Despite the epidemiological significance of Klebsiella pneumoniae, few studies have been performed on the importance of siderophores in the pathogenicity of this species. The frequency of genes encoding proteins involved in iron uptake and their prevalence among saprophytic and clinical strains of Klebsiella pneumoniae in Ilam hospitals should be investigated. In the field of treatment methods, this pathogen was focused on this category.

Hospitals such as pneumonia, urinary tract infections, septicemia, bacterial meningitis and biliary tract infections. Klebsiella is a major cause of nosocomial infections, especially among neonatal and intensive care unit patients with high mortality rates over the past two decades. The incidence of infections caused by multidrug-resistant Klebsiella species has increased. Multidrug resistance in Klebsiella specimens is associated with health care, using only carbapenems as the last possible line of antibiotic defense for infection. Unfortunately, since the early 2000s, Klebsiella strains of multidrug-resistant specimens (MDRs) have begun to produce carbapenemases encoded by plasmids. Which quickly spread to outpatients and hospitalized patients. There are methods for detecting the occurrence of an epidemic from related and unrelated strains as well as comparing specimens at the subspecies level, the most important of which is the epidemiological typing method that brings us closer to this goal. These methods rely on enzymatic cleavage and typing based on PCR and PFGE, which explore the molecular epidemiology of Klebsiella's sample and facilitate cognitive facilitation, as well as isolated strains to better understand the epidemiology of MDR strains. PFGE is often considered the gold standard of other methods, which is a reference method for typing and identifying many organisms, including Klebsiella. The aim of this study was to evaluate the pattern of antibiotic susceptibility as well as PFGE in order to study the molecular epidemiology of Klebsiella strains isolated from hospital clinical patients. Given the importance of MDR in bacteria, the need for new programs to prevent their spread is evident. The misguided policy of overuse of antibiotics, resource constraints, and lack of facilities in hospitals has contributed to these conditions, which have limited the effectiveness of many antimicrobial agents, resulting in significantly reduced treatment choices, which is why Doctors go straight to strong antibiotics, and prescribe the strongest antibiotics. The need for further studies in this field, as well as reducing the use of antibiotics and prescribing antibiotics by physicians is essential due to the results of antibiotic susceptibility testing. In this study, we tried to study the genetic analysis of Klebsiella isolates with multiple drug resistance and antibiotic resistance pattern and determine their types, because to prevent and control infections and to select appropriate treatment, genetic analysis and epidemiological study of these isolates are required. . In this study, by recognizing the clonal relationship of Klebsiella sample isolates and examining their genetic diversity, it can be useful and practical in identifying the source and controlling infections caused by Klebsiella sample.

Enterobacteriaceae, especially Escherichia coli, are the most common cause of nosocomial and community-acquired infections. The emergence and spread of Escherichia coli strains with multiple drug resistance complicates treatment for multiple severe infections.
In this study, pulse field gel electrophoresis (PFGE) method is used for genetic analysis of Escherichia coli strains with multiple drug resistance. Pulse field gel electrophoresis (PFGE) is one of the most useful developments in molecular epidemiology for the past few decades and is now considered the gold standard for molecular typing of microorganisms.
In this study, we have tried to study the genetic analysis of Escherichia coli isolates with multiple drug resistance and the pattern of antibiotic resistance and determine its types, because to prevent and control infections and to select appropriate treatment, genetic analysis and epidemiological study of the region Isolates with drug resistance are required.

A close correlation existed between asymptomatic oropharyngeal colonization of bacterial pathogens and a range of pediatric respiratory disease, also, if this pathogens contain the iron and virulence genes, person to person transmission and subsequent disease of this asymptomatic colonization, Known as a potential risk in children. Thus, in this study we evaluated the frequency of asymptomatic carriers of Neisseria meningitides and Moraxella catarrhalis in healthy childrenIn this study we assess the frequency of Neisseria meningitides and Moraxella catarrhalis in oropharynx of 123 children from 2 to 6 years old in kindergartens of Ilam province, iran, by specific methods including culture and phenotypic diagnosis as well as PCR and Real-time PCR; also iron and virulence genes was evaluated in this bacteria using the PCR method. Finally, statistical analysis was performed to evaluate the colonization of both bacteria in children and the relationship between variables that affecting the frequency of these bacteria. The majority of obtainedbacteria isolated in this study were positive for the presence of virulence and iron genes, given that the presence of these genes for survival of these bacteria in the body is essential; children, who are carriers of these bacteria, were more at risk to develop respiratory diseases by pathogenic bacteria. Finally, due to the study of the relationship between variables and the prevalence of these bacteria appears that most children, who have asymptomatic carriage with Neisseria meningitidis, were present in large families with low economic status, which reflects the importance of the environment and socio-economic level of families in the development of these potentially pathogenic bacteria in the oropharynx of children. So, when these children are placed in high-density locations such as daycare centers, than other children, are more susceptible to asymptomatic colonization of respiratory pathogens and then in the long term may be at risk for respiratory disease.

Acute respiratory infection is one of the primary diseases that causes high morbidity and mortality in children, especially in developing countries, where 70% of deaths are in Africa and Southeast Asia (1). . The latest statistics show that approximately 150 million new cases of pneumonia are diagnosed worldwide each year, more than 90% of which occur in developing countries and 30% of annual deaths in children under 5 years of age. Is (2). In Iran, several studies have been performed on selected populations of children with acute respiratory infections in different parts of the country, which have yielded different results, so an accurate statistical picture of the extent and importance of acute respiratory infections is not available (3). . Acute respiratory infection is often caused by a new invasion or abnormal release (development) of a specific pathogen into the airway, which in addition to the host defense also affects the incidence and severity of infection (4). , Microbiome refers to the overall microbial community and its biomolecules in a defined environment (5). It describes the fundamental role of microbiome in human health (6).
Research based on culture studies and, more recently, culture-independent methods have identified a microbial community in the lower respiratory tract of a healthy individual. (7).
Therefore, studying and informing about the prevalence of bacteria and viruses in pediatric uropharynx and also better understanding of bacterial colonization and their interaction, can be effective in predicting changes in bacterial ecology and design control strategies (including interference in colonization) . In particular, determining the rate of asymptomatic colonization with potential pathogens is important to predict the spread of the disease in the community and to design strategies for prevention and treatment. Therefore, respiratory infections in both developing and developed countries have a heavy economic burden on the family and society. Finally, considering the importance of respiratory infections in our country, in this study we try to investigate the prevalence and frequency of oropharyngeal microbiome in children under 6 years of age in Ilam by culture method and its relationship with the underlying variables to be evaluated. , And thus provide the necessary ground for further studies by researchers in the future

Diarrheal is the second most prevalent causes in under five-year old children worldwide. Shigella spp. which renders dysentery in children are mostly prevailed in underdeveloped countries, where hygiene were poor practices. Shiga toxin is responsible for fatal manifestations of shigellosis including dysentery, hemorrhagic colitis, uremic-hemolytic syndrome, etc. By increasing our knowledge on toxin presence in isolates, effective treatment can be done in future and prevented from further complications. we investigated the frequency of stx1 gene in Shigella spp. (except S. dysenteriae) isolated from diarrheal patients in Ahvaz city, southwestern Iran.
Methods: During a two-year period, 227 Shigella species including 60 S. flexneri, 157 S. sonnei and 10 S. boydii were obtained from diarrheal patients. Upon DNA extraction, PCR was applied for stx1 gene detection in validated isolates.
Findings: Based on our findings, none of the organisms were recognized to be positive regarding the stx1 gene.
Conclusion: The lack of toxin production among the Shigella spp in this area shows that the encoding shiga-toxin phage is not carried by these isolates. However, regarding the emergence of this phage in other territories of the world and the unknown condition for transmission to other species, we can’t make an absolute decision on its absence in this region.

Among widespread cause in gastrointestinal disorders we can mention shigella flexneri . Because of its complicated virulence factors, it seems to be good to investigate S. flexneri genome and recognize regulatory sequences. Toxin antitoxin systems is one of regulatory sequence that have important role in cellular process so we choose mazEF and relBE for more analysis and its effect on virulence factors VirF, IpaB and shigella enterotoxin1.

Background

Gastroesophageal reflux disease (GERD), which leads to acid reflux into the esophagus, is a common gastrointestinal disorder. Several studies have shown the prevalence of GERD in Iranian population, but their evidence is contradictory. Therefore, the present study was conducted to investigate the epidemiology of GERD in Iran.

Methods

The entire steps of this systematic review and meta-analysis were based on the MOOSE protocol, and the results were reported accordance with the PRISMA guideline. This review is registered on PROSPERO (registration number: CRD42020142861). To find potentially relevant published articles, comprehensive search was done on international online databases Scopus, Science Direct, EMBASE, PubMed/Medline, CINAHL, EBSCO, Cochrane Library, Web of Science, Iranian online databases and the Google Scholar search engine in June 2019. Cochran test and I2 index were used to assess the heterogeneity of the studies. Data were analyzed using Comprehensive Meta-Analysis software ver. 2. The significance level of the test was considered to be P <  0.05.
