Introduction: Nosocomial infections are a major worldwide public health problem. These infections are related to a high rate of mortality and morbidity in hospitalized patients. The aim of this meta-analysis survey was to evaluate the prevalence of nosocomial infections among hospitalized patients in Iran.

Methods: We searched Iranian electronic databases (SID, IRANMEDEX, IRANDOC, and MAGIRAN) and MEDLINE for studies that were related to nosocomial infections in Iran. Random effect meta-analysis was applied to pool the prevalence of nosocomial infections as well as the frequency of bacterial pathogens. Data has been interpreted considering, rate of nosocomial infections, frequency of nosocomial pathogens and type of nosocomial infections.

Results: Twenty eight studies from 1997 to 2011, involving 22775 patients were included in the analysis. Among them, 3267 patients (28.8%) were infected by nosocomial pathogens (CI 95%: 23.8%-33.7%). Burn wound infections were the most prevalence infection (53.2%) followed by respiratory infection (49.5%), bacteremia (20.7%), Urinary tract infection (19.7%), and surgical wound infections (15.2%). Among nosocomial pathogens, acinetobacterspp (22%) was the most frequent pathogen followed by staphylococcus aurous (21.3%), Klebsiella spp. (20.4%), E.coli (20.3%) and pseudomonas aeruginosa (20.2%).

Conclusion: According to the results of this meta-analysis, the prevalence of nosocomial infections was high; therefore an intervention program for control of hospital acquired infections across Iran is needed.

ABSTRACT
Introduction: Pain is the key concern of women after caesarean
delivery that may interfere with breastfeeding.
Aim: The aim of this study was to assess effect of ketofol
(ketamine/propofol combination) on pain and complication after
caesarean delivery under spinal anaesthesia.
Materials and Methods: In this randomized double-blind
clinical trial, 92 parturient scheduled for elective caesarean
delivery under spinal anaesthesia were included. The simple
random sampling method was used to place subjects in four
groups of ketamine (0.25 mg/kg), propofol (0.25 mg/kg), ketofol
(25 mg ketamine plus 25 mg propofol) and placebo (saline).
The drugs were administered intravenously immediately after
clamping the umbilical cord. Visual Analog Scale (VAS) was used
to determine the intensity of pain. Complications after surgery
including shivering, nausea and vomiting as well as onset of
breastfeeding were recorded.
Results: The mean score of pain, morphine consumption and
time of breastfeeding in the ketofol group were significantly
lower than other groups at various intervals (p<0.05, p<0.001).
The frequencies of shivering, nausea, vomiting,
retention and
pruritus in the ketofol group were significantly
lower than other
groups (p<0.001, p<0.05)
Conclusion: The effective role of ketofol on reducing pain and
complication after caesarean delivery indicated that it can be
considered as a safe and alternative drug in these patients.

Introduction: Hypertension is one of the chronic health problems across the world and raises significant global responsibility in developed and developing countries. This study was to examine the effect of education on the lifestyle of patients with hypertension.
Methodology: This study was a case-control intervention study on 86 patients with hypertension that were selected by simple random sampling from the rural regions of Aligoudarz County in Lorestan Province. Before the intervention, both groups completed the standard questionnaire of HPLP II; and two months after the treatment, both groups completed the same questionnaire. The results were analyzed using the SPSS software, t-test, and Chi- square.
Results: The age mean and the standard deviation in both case and control groups, were respectively 59.95 ± 7.9 and 64.51 ± 9.2 years old. The mean for the total lifestyle scores had a significant increase in the case group than that of the control group (P <0/05). As well, the average scores for the three dimensions of physical activity, nutrition, and stress management after the educational intervention in the case group compared to the control group showed a significant increase (P <0/05).[مرتضی1]
Conclusion: Given the relationship between lifestyle and hypertension, it seems that implementing educational programs in the field of nutrition, physical activity, and stress are essential to the improvement of the knowledge and modification of the behavior among patients with hypertension.

Abstract
Introduction: Intrauterine growth retardation is a physiological and pathological process resulting in a slow or restricted fetal growth, and not only increases fetal mortality, but also is associated with adverse outcomes during infantile and adulthood. Therefore, the present study aimed to determine the prevalence and risk factors associated with IUGR in Ilam province in one year period.
Materials and Methods: In this study, the prevalence of intra uterine growth retardation (IUGR) was measured by a cross-sectional method among all neonates was born during 2014 in ilam province. Then, by designing a case- control study 781 newborns, including 247 cases and 534 controls were enrolled. Cases were characterized by a birth weight below than 10th percentile for the mean weight, standardized for GA and fetal sex and controls were characterized by a birth weight above the 10th percentile were selected via a simple randomization method. The required information was obtained from hospital records. Data analysis was performed using chi-square test and logistic regression model with SPSS statistical software version 16.
Results: Among 9053 neonates، the prevalence rate of neonates born with IUGR in Ilam province during the study period was 2.8%. The median of gestational age in this study was 39 weeks and female/ male ratio of neonates was 1.09. There was a significant relationship between IUGR and multiple pregnancy (OR:24.09; P:0.0001), history of prior fetal death (OR:16.49; P:0.02), mother's place of life (OR:1.86; P:0.001), autoimmune diseases (OR:7.62; P:0.03), kidney disease (OR:18.94; P:0.01), preeclampsia (OR:5.25; P:0.001), hyperemesis gravidarum (OR:15.54; P:0.01), treated hypothyroidism (OR:0.2; P:0.009) and mode of delivery (OR:0.51; P:0.001).
Conclusions: It seems that the prevalence rate of neonates born with IUGR in Ilam province was in accordance with global standards. The results showed that multiple pregnancy, history of prior fetal death, preeclampsia, hyperemesis gravidarum, autoimmune and kidney diseases and rural residence increase the chance of intrauterine growth retardation.

Background: Achieving professional identity is one of the research priorities, and considering the importance of professional attitude
among student nurses, it is necessary to identify a scale that is able to measure their achievement in professional attitude.
Objectives: The present study was conducted with the aim of translation and psychometrics of instrument of professional attitude
for student nurses (IPASN) scale.
Methods: In this cross-sectional study, the translation and psychometrics of “instrument of professional attitude for student nurses
scale” was performed based on the model of Wild et al. The third to eighth semester nursing students of Ilam University of Medical
Sciences comprised the research population who were 300 students. After translation and retranslation, the editorial comments
of the scale designer were applied. Then, content validity, face validity, confirmatory factor analysis, internal consistency, and testretest
reliability of the Persian version were calculated. Data were analyzed using SPSS software version 20 and EQS6.1.
Results: The confirmatory factor analysis of the 28-item scale with its 8 sub-scales was confirmed by deleting the statement 7 and
moving the items 10, 15, and 18. The reliability of the internal consistency was calculated to be= 0.89 for the total scale, and (0.89),
(0.45), (0.67), (0.69), (0.69), (0.73), (0.70), and (0.93) for the sub-scales, respectively. Pearson’s correlation coefficient was r = 0.79 for
test-retest reliability (P < 0.005).
Conclusions: This study shows that the modified Persian version of the instrument of professional attitude for student nurses scale
with 27 statements is valid and reliable, and can be used to assess the nursing students towards their professional life.

OBJECTIVES
Little information exists on the association between comorbidities of mental disorders and suicidal ideation in developing countries. The current study examined the relationship between the presence of comorbid mental disorders and suicidal ideation in the adult population.
METHODS
This cross-sectional study was conducted using the cluster random sampling method in 3 steps. Data were collected from a household assets survey and the self-administered 28-item General Health Questionnaire as first step in screening, and the Persian version of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition - Text Revision was used in the second stage to determine the prevalence of mental disorders. Bivariate and multivariate analysis were used to investigate the associations between mental disorders and suicidal ideation.
RESULTS
Of the 763 participants, 199 (26.1%) had 1 or more mental disorder. Forty-two (71.4%) subjects with comorbidities had a history of suicidal ideation, whereas 59 (7.7%) of all participants had a history of suicidal ideation. We found that major depressive disorder and obsessive-compulsive disorder were the most predictive of suicidal ideation in both sexes. The odds ratio for suicidal ideation associated with having 3 comorbid disorders was 2.70 (95% confidence interval [CI], 1.40 to 14.12) in males and 3.06 (95% CI, 1.25 to 15.22) in females.
CONCLUSIONS
Consistent with pervious data, our results confirmed that mental disorders and comorbidities of mental disorders were important predictors of suicidal ideation. Our findings are very useful for applied intervention programs to reduce the suicide rate in regions in which it is high.

Background: An association between cancer survival and socioeconomic status (SES) has been evaluated for many different cancers but calculating socioeconomic inequality in survival is very lake. In this study we aimed to determine associations between socioeconomic inequality and survival risk factors in patients with gastric carcinoma (GC).
Methods: In this cross-sectional study, we enrolled 235 patients with confirmed of GC. SES data were retrieved from three sources pathological records, official death certificates and telephone interviews. polychoric correlation matrix was used to reduce the number of variables. Inequality measured by concentration index (CI) and we decomposing CI to determine contribution in inequality. All analysis was performed by standard statistical software, STATA (Version 11.2).
Results: The overall CI for late diagnosis (2 stage onwards) and positive history of smoking were -0.020 (95% CI = -0.041– 0.004) and -0.105 (95% CI = -0.110– -0.076), respectively. Results of decomposition shows past medical history of gastrointestinal diseases (29%) and history of smoking (18%) have a largest contribution in inequality in GC survival.
Conclusion: Results of this study showed risk factors in GC survival such as smoking, having a past medical history of gastrointestinal diseases and late diagnosis is more prevalent among people of lower SES.

Chronic heart failure is the most common form of heart disease, especially in the elderly over 65 years of age, and the most common cause of their hospitalization. Also, the rate of readmission and the amount of treatment costs due to hospitalization in these patients are very high. Congestive heart failure (CHF) results in readmission to the hospital within 60 days of discharge. Evidence suggests that up to 1.3 hospitalizations due to CHF have not been compensated, which may be preventable. Self-care in heart failure indicates issues such as diet and medication, sodium and fluid restriction, amount of activity allowed, daily weight, follow-up and decision-making for appropriate treatment measures during severe illness. Heart failure patients need training and support in medication regimen, how to use Medications, diet, activities, identifying the symptoms of heart failure and worsening symptoms and taking appropriate action when symptoms occur. Patients with heart failure should be informed about the symptoms of the disease as well as the importance of following medication instructions and limiting water and salt intake. Improving self-care behaviors helps patients gain more control over their lives and cope with their social functioning. The aim of this study was to determine the effect of education on self-care in patients with chronic myocardial infarction.

The consolidation of information entering the brain underlies the phenomena of learning and memory. This information can be stored in many parts of the brain, including the new cortex and the hippocampus. The hippocampus is the area in the middle temporal lobe that plays a key role in stabilizing memory and spatial learning in mammals; So that the destruction of this part disrupts the use of memory.
Vitamin D deficiency increases the risk of cognitive and dementia in the elderly.
However, other studies have shown that deletion of the vitamin D receptor gene has little effect on memory activity.
The structural and functional health of neurons is very important in the formation of memory and learning phenomena, and the role of this vitamin in maintaining the health of neurons has also been shown. Vitamin D protects the cortical cells of the brain against damage to neurons by increasing the expression of its receptor messenger RNA.
Considering the mentioned cases and the increasing prevalence of osteoporosis (especially in women) due to calcium and vitamin D deficiency in Iran, examining the relationship between this deficiency and memory and learning can be important in better understanding their pathogenesis. The aim of this study was to investigate the relationship between vitamin D deficiency and memory and statistical learning.
This research will be done systematically and meta-analysis on published studies in this field. For the findings of this study, articles published in the journals Prequest, Science Direct, PubMed, Google Scholar, Magiran, SID and Iran Medex will be used. The keywords used (main and sub-variables), search strategy (as a search in the title), the distance of the searched years (2015-2001) will be the most important items observed in the search.

Background: Numerous studies have focused on investigating the role of iron serum in the emergence, and development, of cardiovascular diseases (CVD); these studies have produced varying results. The present systematic meta-analytical review was aimed at investigating the relationship between the level of iron serum and the development of CVDs.
Methods: The present study conducted a systematic meta-analytical review of the articles published between 7110-7102. In so doing, two independent researchers used appropriate keywords to search medical databases of SID (Scientific Information Database), Mag Iran, Iran doc, Med lib, Iran med ex, Direct Science, Scopus, Google, and Pub Med; collected data, then, was analyzed using SPSS 0.61.
Results: According to the results of the meta-analysis study, mean serum iron was 7562.±75622
μmol/L in heart disease group; thus, there turned out to be a significant difference, with a confidence level of %29 (P=.110), between the groups with and without heart disease in this regard. The mean level of ferritin serum was 0126127±.16717 μmol/L in the heart disease group, indicating a significant difference between two groups (P=.110).
Conclusion: As an essential element, iron is needed to maintain the natural structure of cells and cell viability; moreover, regardless of other risk factors, ferritin, as the representative of total iron in the body, turned out to be more common in cardiovascular patients compared to the group without the disease, which can be attributed to the probable role of iron storage in causing CVD.

The results of the present study showed that administration of hydroalcoholic extract of chickpea in hypercholesterolemic rats caused a significant decrease in serum levels of triglycerides, cholesterol and LDL-c and a significant increase in serum levels of HDL-c. The extract also had the ability to reduce inflammatory factors so that its administration significantly reduced serum levels of IL-6, CRP, Fibrinogen and TNF-α in hypercholesterolemic rats. It is possible that the mechanism of this plant is due to the observed hypolipidemic and anti-inflammatory effects caused by its polyphenolic compounds. In addition, the results of the present study indicated that the protective effect of hydroalcoholic extract of chickpea is dose-dependent, as the highest effectiveness was observed at a dose of 75 mg / kg. According to the above, it can be concluded that the dose / kg 75 Hydroalcoholic extract of chickpea has the potential to be used as a natural product for the management of cardiovascular disease

Aim: Patients with type 2 diabetes mellitus (T2DM) have an increased risk of bone fractures. A variable increase in fracture risk has been reported depending on skeletal site, diabetes duration, study design, insulin use, and so on. The present meta-analysis aimed to investigate the association between T2DM with fracture risk and possible risk factors.
Methods: Different databases including PubMed, Institute for Scientific Information, and Scopus were searched up to May 2016. All epidemiologic studies on the association between T2DM and fracture risk were included. The relevant data obtained from these papers were analyzed by a random effects model and publication bias was assessed by funnel plot. All analyses were done by R software (version 3.2.1) and STATA (version 11.1).
Results: Thirty eligible studies were selected for the meta-analysis. We found a statistically significant positive association between T2DM and hip, vertebral, or foot fractures and no association between T2DM and wrist, proximal humerus, or ankle fractures. Overall, T2DM was associated with an increased risk of any fracture (summary relative risk =1.05, 95% confidence interval: 1.04, 1.06) and increased with age, duration of diabetes, and insulin therapy.
Conclusion: Our findings strongly support an association between T2DM and increased risk of overall fracture. These findings emphasize the need for fracture prevention strategies in patients with diabetes.

Urinary tract infections are one of the most common bacterial infections in humans of all ages and the use of urinary catheters is one of the most important underlying factors, especially when they remain for a long time. Complications from catheter use can sometimes lead to serious risks, such as "blood infections" or death. The aim of this study was to determine the prevalence of bacterial urinary tract infections after catheterization and to determine the pattern of antibiotic resistance of bacteria isolated from patients.
. At least 2 urine samples were taken from each patient, before and after catheterization, and after transferring them to the laboratory, in order to count the colonies, they were inoculated to appropriate culture media by standard methods. By isolating and identifying the bacteria, a suspension was prepared from each known colony and tested for susceptibility or resistance to eight different antibiotics by "disk-diffusion" (and the Kirby-Bauer method).
The results showed that out of 87 patients who did not have any signs or symptoms of urinary tract infection before receiving the catheter, 38 (43.6%) became infected with bacteriuria at the end of catheterization, while the highest rate of bacteriuria was in the age of 30-39. Year (28.9%) and the lowest was in the ages of 60-69 years (2.6%). The results of culture of urine samples led to the isolation of 50 bacterial strains, among which, E. coli with 17 strains (34%) the highest rate and, Kl. pneumoniae, Staph. aureus Edwardsiella tarda and Ent. sakazakii had the lowest percentage (2%) of isolates. Escherichia coli, Enterobacter aerogenesis and Klebsiella rhinoschromatis showed the highest resistance to ampicillin, penicillin and cephalexin antibiotics and the lowest resistance to nalidixic acid, gentamicin and nitrofurantoin. Staphylococcus epidermidis also showed the highest resistance to penicillin and ampicillin (100% each) and the lowest resistance to gentamicin (66.7%) and cotrimoxazole and nitrofurantoin (50%). Other isolated bacteria showed varying degrees of resistance or susceptibility to the antibiotics tested.
The results of this study indicate an increase in the resistance of these bacteria to ampicillin and penicillin G, the need to replace them with other antibiotics if it is possible not to use urinary catheters.
Evaluation of the incidence of urinary tract infections in hospitalized patients and the effect of standard bladder catheterization training on the knowledge and attitude of nurses working in intensive care units of Shahid Mostafa Khomeini Hospital in Ilam in 2017
Urinary tract infections are one of the most common bacterial infections of humans of all ages and use
Urinary catheters are one of the most important underlying factors, especially when they remain for a long time. Complications from catheter use can sometimes lead to serious risks, such as "blood infections" or death. The aim of this study was to determine the prevalence of bacterial urinary tract infections after catheterization and to determine the pattern of antibiotic resistance of bacteria isolated from patients.
In this study, a total of ٢٢٦ ٢٢٦ samples of ١١٩ patients who underwent different surgeries for different parts of the two
Razi and Golestan hospitals in Ahvaz were referred and catheterized and studied. At least from each patient
Urine samples were taken before and after catheterization, and after transferring them to the laboratory, in order to count the colonies, they were inoculated into suitable culture media by standard methods. By isolating and identifying the bacteria, a suspension from each colony was prepared in terms of susceptibility or resistance to ٨ (known as Kirby-Bauer and tested by "disk-diffusion") and tested with various antibiotics. Of the 4 patients who (٤٣% (before the catheter received no signs or symptoms of urinary tract infection) (at the end of the catheterization) became infected with bacteriuria, while the highest rate / did not have, 3
٢ percent). Results: 7.1 years (1-4%) and the lowest at the age of 2.7 years (1-bacteriuria at ages 2 with 5 strains of E. coli) were cultured urine samples leading to the isolation of 2 strains of bacteria, among which The lowest Ent. Sakazakii and Edwardsiella tarda, Kl. Pneumoniae, Staph. Aureus (1%) had the highest amount and (1%) the isolates. Escherichia coli, Enterobacter aerogenesis and Klebsiella rhinoschromatis showed the highest resistance to ampicillin, penicillin and cephalexin antibiotics and the lowest resistance to nalidixic acid, gentamicin and nitrofurantoin. Staphylococcus epidermidis also showed the highest resistance to penicillin and ampicillin (با% each) and the lowest resistance to gentamicin ٦٦%) and cotrimoxazole and nitrofurantoin (٥٠%). Other isolated bacteria showed different values with% of resistance or sensitivity to the antibiotics tested.
Necessity, G The results of this study indicate an increase in resistance of these bacteria to ampicillin and penicillin
Replace them with other antibiotics if possible by not using urinary catheters.

An expression of beliefs and values is called philosophy, which can be expressed individually or jointly between group members, such as a nursing school, and nurses need to be aware of their own nursing philosophy and that of the institution when hiring, nursing activities, and communicating with their patients. Nursing philosophy provides a perspective on practice, knowledge and research.
It also plays an important role in the process of production and development of knowledge in the field of nursing and professionalization. Therefore, the purpose of nursing philosophy is to shape and guide the performance of nurses that arise in the shadow of nursing theories. Since better knowledge and application of philosophy for all nursing disciplines requires knowledge of basic philosophical concepts and because this issue is not very clearly addressed in undergraduate and graduate courses, the researcher decided based on experiences and using articles and resources. Existing valid science to study different aspects of philosophy and its role in nursing.

Patient safety culture is the first necessary step to reduce medical errors and improve the patient's condition. For this reason, the present study was conducted to determine the status of patient safety culture in hospitals in Ilam in 2016.
Materials and Methods: In this descriptive-analytical study, 150 nurses in Ilam were selected by random sampling. The questionnaire used was the standard Hospital Survey On Patient Safety Culture questionnaire whose validity and reliability had been confirmed in previous studies. Data were analyzed using SPSS software version 17.
Results: The findings of the present study showed that the safety culture of the nurses was moderate and there was no statistically significant difference between the studied hospitals in terms of patient safety culture (p <.05). Among the dimensions related to patient safety culture, the dimension of extra-organizational teamwork and non-punitive response had the lowest mean and the dimensions of organizational learning and general understanding of safety had the highest mean.
Conclusion: Considering the importance of patient safety culture, it is necessary to take the necessary interventions to improve the patient safety culture among nurses, especially in the two dimensions of extra-organizational teamwork and non-punitive response that have the lowest mean.
