Helicobacter pylori (H. pylori) is the causative agent for upper gastrointestinal diseases. The presence of the cagA, at least with one repeat of EPIYA-C, and the vacA with s1-i1 polymorphisms are likely to be associated with increased gastric cancer risk. Aim: The aim of study was to determine the genotype of cagA and vacA and their usefulness as a predictive factor in upper gastrointestinal patients in Ilam, Iran. Cytotoxin-associated gene A (cagA) is another important factor related to H. pylori infection and development of GC [7]. Upon entrance of CagA to host cells by T4SS, several repeats of Glu-Pro-Ile-TyrAla (EPIYA) motifs have been phosphorylated by Src and Abl family kinases, whereas other CagA molecules remain un-phosphorylated, host signaling pathways disrupted and lead to abnormal proliferation of gastric epithelial cells [8, 9]. In addition, phosphorylation of the tyrosines located in the EPIYA motif of cagA induces activation of a receptor tyrosine kinase (RTK) signaling cascade [7]. It has been shown that activated RTK enhances cell proliferation and invasiveness of
the cell which might eventually result in cancer development [7]. cagA is generally accepted as a member of cag PAI (pathogenicity island) which codes a T4SS [10].

Purpose
The aim of this randomized controlled study was to determine and compare the effect of a 12 weeks RT period with 2 different periodization models on serum interleukin (IL)-
7, IL-15, insulin like growth factor-1 (IGF-1) and insulin levels and functional capacity.
Methods
Twenty eight untrained women 28-43 years with abdominal obesity (waist ˃80 cm) were randomly assigned to: linear periodization (LP, n=10), nonlinear periodization (NLP,
n=9) and control (CON, n=9) groups. LP and NLP performed RT protocols 3 days a week for 12 weeks. Blood samples were taken in fasted state before and three days
after the training period and were analyzed for the biochemical variables.

Introduction: Every day, a large number of patients with severe heart pain or similar cases leading to heart attacks, go to the emergency room in the hope of survival from dangerous and deadly conditions. Knowledge and practice of cardiopulmonary resuscitation of nurses working in intensive care units of teaching hospitals in Ilam in 2019.
Materials & Methods: In this quasi-experimental study, 40 nurses were studied by simple random pretest-posttest method. To collect the data, a questionnaire consisting of 20 researcher-made questions was used that measured the knowledge and standard checklist of nurses' performance. Inclusion criteria included informed consent to participate in the study, at least one year of experience in care wards. Special requirements and at least a bachelor's degree in nursing and exclusion criteria were withdrawal from continuing studies and not participating in educational programs. Data were analyzed using SPSS V.19 software using descriptive statistics (frequency, mean, standard deviation) and inferential statistics (correlation, linear regression and Chi-square test, as well as ANOVA analysis of variance). Significance level was considered for the tests (P <0.05).
Results: The sample consisted of 40 people, of which 31 (77.5%) were women, 27 (67.5%) were project personnel and 13 (32.5%) were formal employment personnel, and Finally, 39 (97.5%) were experts and one (2.5%) was a master of nursing. The mean scores of knowledge before training were 10.8. 2.68 and after training were 16 2 2.77. The mean performance score (before training) was 13.23 ± 2.1 and after training was 14.78. 2.2.The results of the study showed that the amount of knowledge
And the participants' performance about cardiovascular resuscitation training after the intervention has increased more than before, and this difference is statistically significant (P <0.0001).
And the participants' performance about cardiovascular resuscitation training after the intervention has increased more than before, and this difference is statistically significant (P <0.0001).
Discussion and Conclusion: The results showed that most nurses perform CPR based on old protocols, so it is necessary to express these changes during retraining. In addition, due to the decrease in the level of knowledge and skills, it is necessary to repeat these courses over time.

Although there have been many advances in the treatment of heart disease in recent decades, the prevalence of heart failure is increasing. The American Heart Association reports that about 7.3 percent of all deaths from heart disease are often due to heart failure. (1) Heart disease often leads to chronic heart failure, an incurable disease with severe and progressive fatigue. , Intolerance to exercise, fluid retention and shortness of breath are characterized (2).
Heart failure is one of the most common cardiovascular disorders and is considered as a chronic, progressive and debilitating disorder. Its prevalence increases with age, so that in the United States, about one percent of people over the age of 50 and about ten percent of the elderly over the age of 80 have heart failure. On the other hand, progress in treatment and surgery has caused patients who survive death from myocardial infarction to subsequently develop heart failure (3) Increasing trend of heart failure due to complications of infectious, inflammatory, vascular and valvular diseases Heart disease is also a major health problem and an epidemic in the United States, with 5 million people with heart failure in the United States, and about 500,000 new cases are added each year, and are expected in the next 30 years. Double the existing statistics (4) Research shows that the quality of life of heart failure patients is lower than the general population and also compared to other chronic patients. (5)
The US Food and Drug Administration (FDA) approved Corlanor, a generic drug called Ivabradine, in 2015 to reduce the severity of heart failure (chronic heart failure) in 2015. It is used in patients with symptoms of persistent heart failure (normal heart rate is at least 70 beats per minute).
Cost-effectiveness analysis of Ivabradine as a new strategy in the treatment of heart failure in the field of non-communicable diseases in comparison with standard care can play an important role in informing decision makers about reimbursement of costs, pricing of medical interventions, providing clinical guidelines Cost-effective use of next-generation drugs, targeted interventions, and the production of scientific evidence to make policy decisions will ultimately lead to cost management, efficiency, and optimal allocation of the health system's limited financial resources in the field of cardiovascular disease economics. Given the high burden of cardiovascular disease, limited financial resources in different countries and the share of their GDP in the health system, conduct more comprehensive studies to eliminate these differences in analysis due to limited resources. It seems necessary to introduce a dominant option in the field of economic evaluation based on studies conducted worldwide. Therefore, the present study systematically reviews the economic evaluation of Ivabradine in comparison with other common treatments, focusing on patients with heart failure.

Objectives: nowadays, the emergence of antibiotic resistance has surpassed the discovery of new antibiotics. One of the strategies that cause pathogenic bacteria, such as S.aureus, by which develop chronic and treatment-resistant infections is biofilms formation. Traditionally use of Espand, has a long history in Iran. In this study, we exanimate the anti-biofilm property of the alcoholic extract of espand on S.aureus isolated from burn patients.
Methods: 42 isolates of S.aureus were divided from the wounds of burn patients and assess their ability for biofilm formation then exanimated for determination of biofilm-related genes, icaD and icaA. Toxicity of this extract was measured on Vero cells by determining IC50. Also >IC50 concentration were investigate on biofilm and icaA and icaD expression.
Results: Our results in this study showed that S.aureus is able to form different degrees of biofilm. Espand is toxic for Vero cells at the concentration of 35 μg / ml and can easily inhibit Staphylococcus aureus biofilm at the concentration of 9 μg / ml.
Conclusion: Espand is one kind of common Iranian herbal medicin. This study showed that Espand have inhibitory effect on S.aureus biofilm and transcript levels of icaA, icaD were significantly decresed upon the addition of P.harmalla, but the determination of pharmacokinetic details and further studies on laboratory models can approveits effectiveness, to allow us to use it as an alternative in the treatment of infection caused by the pathogen.

Nurses have deep relationships with patients. They encounter a lot of situations in their work environment during the day. These situations require ethical decisionmaking. The ability of ethical reasoningis necessary for moral decision making. This study aimed to compare the ability of ethical reasoning of nursing students and nurses working in the teaching hospitals affiliated to Ilam University of Medical Sciences. This descriptive-analytic study was conducted in 2017 in the teaching hospitals affiliated to Ilam University of Medical Sciences. The sample consisted of 122 third- and fourth year-nursing students at Ilam Nursing and Midwifery Faculty as well as 380 nurses who were selected randomly. Data were collected using nursing dilemma test questionnaire. The descriptive-analytic statistical tests and SPSS (version 24) were used for data analysis. Findings showed that there was no significant difference between mean scores of ethical reasoning of nursing students and nurses (p = 0.803). Moreover, there was no significant relationship between the level of ethical reasoning and individual characteristics of nursing students and nurses such as age, gender, educational level, marital status, work experience and working area of employed nurses (P> 0.05). Educational programs such as workshops and continuous education courses will be useful in developing ethical reasoning for nursing students and nurses.

Abdominal aortic aneurysm is an abnormal localized increase in the size of the aorta caused by progressive weakening of the aortic wall. The abdominal aortic aneurysm typically refers to the abdominal aortic diameter greater than three centimeters. (1) Aortic aneurysms can spread to any part of the aortic length, but three-quarters of them are in the abdominal aorta.
The overall prevalence of abdominal aortic aneurysm in men ranged from 1.9% to 18.5% and in women from zero to 4.2%. (4,3) The prevalence of abdominal aortic aneurysm in men from four to six times More than women have been reported (5) and its incidence increases with age. (6) In Western countries, the prevalence of abdominal aortic aneurysms has been reported between 1.8-3.9% in men and 1-2.2% in women. (7) In Iran, the prevalence of abdominal aortic aneurysm in the population of 65-year-old men was 10% (8) and the frequency of abdominal aortic aneurysm in people with coronary artery disease was 14%. (9)
Rupture of the abdominal aortic aneurysm is the 13th leading cause of death in the United States and the 10th leading cause of death among older men (10:11) and the annual leading cause of death is 9000. (12) About 30,000-40,000 asymptomatic patients to prevent Ruptures undergo elective abdominal aortic aneurysm, where postoperative mortality will be between 2-8% (13, 14)
Cost analysis of endovascular efficacy compared to open surgery repair in the treatment of abdominal aortic aneurysm rupture can play an important role in informing decision makers about reimbursement of care costs, pricing of treatment interventions, providing cost-effective clinical guidelines on use These methods provide targeted interventions and the production of scientific evidence to make policy decisions and ultimately lead to cost management, efficiency and optimal allocation of limited financial resources of the health system in the field of cardiovascular disease economics. Given the burden of cardiovascular disease, limited financial resources in different countries and the share of their GDP in the health system, conduct more comprehensive studies to eliminate these differences in analysis due to limited health resources. And treatment and introduction of the dominant option in the field of economic evaluation based on studies conducted worldwide seems necessary, so the present study is a systematic review by comparing endovascular repair compared to open surgery and focusing on patients with Abdominal aortic aneurysm rupture and evaluated the survival rate, quality improvement and desirability in patients with the results of the study
