fa تعیین اثرحفاظتی عصاره Hypericum در سمیت کلیوی ناشی از سیس پلاتین در رات های نر Protective effects of pretreatment or concomitant treatment with Hypericum extract on renal function and renal toxicity in cisplatin-induced nephrotoxicity سایر.... آزمایشگاهی سیس پلاتین یک داروی ضد سرطان است، که اسم کامل آن (CDDP) cis-diamminedichloroplatinum می باشد. سیس پلاتین در عمل همانند داروهای آلکیله کننده است که در  همه مراحل چرخۀ سلولی، سلول ها را از طریق تشکیل اتصالات متقاطع بین زنجیره های DNA و مهار نمودن سنتز آن ها تخریب می کند و همچنین با تولید مواد اکسیدانی منجر به آسیب بافت می گردد. این دارو در کلینیک ارزش فراوانی دارد زیرا فعالیت ضد توموری آن قوی است و به ویژه در درمان سرطان های پیشرفته و تومورهای بدخیم به کار می رود. سیس پلاتین علیرغم اثرات درمانی دارای عوارض جانبی متعددی نیز می باشد. از جمله مهمترین عوارض این دارو نفروتوکسیسیتی(مسمومیت کلیوی ) است و مصرف آن به علت عوارض جانبی زیادی که روی اعضاء مختلف به ویژه کلیه ها به جا می گذارد دارای محدویت است. تجمع سیس پلاتین در کلیه نسبت به سایر ارگان ها بیشتر است و کلیه به عنوان ارگان اصلی برای دفع آن محسوب میشود . این ماده از طریق فیلتراسیون گلومرولی در فیلترای گلومرولی ظاهر می شود و فرآیند بازجذب روی آن صورت نمیگیرد . استفاده از سیس پلاتین منجر به بالا رفتن سطح کراتینین، ازت اوره <st1:citation w:st="on">( BUN )</st1:citation> خون وهمچنین باعث نکروز حاد سلولی درتوبولهای کلیوی به ویژه لوله های پیچیده نزدیک و نیز آپوپتوز سلولی و آسیبهای بافت بینابینی به شکل فرآیندهای آماسی می گردد که نهایتاً منجر به کاهش فیلتراسیون گلومرولی و بروز سمیت حاد کلیوی میشود. چندین فاکتور همانند گونه های راکتیو اکسیژن (ROS)، سنتز سیتوکاین ها و کموکاین ها در پاتولوژی صدمات کلیوی شرکت دارند. درمان با CDDP سطح مالون دی آلدئید (malondialdehyde, MDA) را افزایش و renal glutathione(GSH) را کاهش می دهد در حالی که عوامل آنتی اکسیدانی سطحMDA را کاهش و GSH را افزایش می دهد همچنین از افزایش کروم ناشی از CDDP هم جلوگیری می کنند. CDDP همچنین باعث کاهش فعالیت کاتالاز ، superoxide dismutase(SOD) و گلوتاتیون پراکسیداز میشود که درمان های آنتی کسیدانی آن را جبران می کند. مشاهدات نشان داده که نفروتوکسیسیتی ناشی از رادیکالهای مختلف اکسیژنی مثل آنیون سوپراکسید ، پراکسید هیدروژن و هیدروکسیل می باشد. از طرفی اجزای فنولی و به خصوص فلانوئید ها منابع قدرتمند آنتی اکسیدانی هستند. بسیاری از گیاهان حاوی فلانوئید ها هستند. گزارش شده است که گیاه هیپریکوم دارای اثرات نوروپروتکتیو و آنتی اکسیدانی است. هیپریکوم محتوی اجزای بیواکتیوی همانند: hypericin و Hyperphorin و flavonoids و مشتقات  xanthan و biapigenin است. این گیاه در اختلالات ایسکمی روده و یا کبدی اثرات آنتی اکسیدانی و ضد التهابی داشته است. مطالعه ای روی اثرات حفاظتی پیش درمانی با عصاره هیپریکوم(و یا به صورت استفاده همزمان) در نفروتوکسیسیتی ناشی از سیس پلاتین در دسترس نیست. Cisplatin is an anticancer drug, whose full name is (CDDP) cis-diamminedichloroplatinum. In practice, cisplatin is the same as alkylating drugs that destroys cells at all stages of the cell cycle by forming cross-links between DNA chains and inhibiting their synthesis, and also leads to tissue damage by producing oxidant substances. This drug has great value in the clinic because its antitumor activity is strong and it is especially used in the treatment of advanced cancers and malignant tumors. Despite its therapeutic effects, cisplatin has many side effects. Among the most important side effects of this drug is nephrotoxicity (kidney poisoning) and its use is limited due to the many side effects it leaves on different organs, especially the kidneys. Accumulation of cisplatin in kidney is more than other organs and kidney is considered as the main organ for its elimination. This substance appears in glomerular filters through glomerular filtration and the reabsorption process does not take place on it. The use of cisplatin leads to an increase in the level of creatinine, blood urea nitrogen (BUN), and also causes acute cell necrosis in the renal tubules, especially the proximal convoluted tubules, as well as cell apoptosis and interstitial tissue damage in the form of edematous processes, which ultimately leads to a decrease in filtration. glomeruli and the occurrence of acute renal toxicity. Several factors such as reactive oxygen species (ROS), synthesis of cytokines and chemokines are involved in the pathology of kidney injuries. Treatment with CDDP increases the level of malondialdehyde (MDA) and decreases renal glutathione (GSH), while antioxidant agents decrease the level of MDA and increase GSH, and also prevent the increase of chromium caused by CDDP. CDDP also decreases the activity of catalase, superoxide dismutase (SOD) and glutathione peroxidase Antioxidant treatments may compensate for it. Observations have shown that nephrotoxicity is caused by different oxygen radicals such as superoxide anion, hydrogen peroxide and hydroxyl. On the other hand, phenolic components and especially flavonoids are powerful sources of antioxidants. Many plants contain flavonoids. Hypericum plant has been reported to have neuroprotective and antioxidant effects. Hypericum contains bioactive components such as hypericin, hyperphorin, flavonoids, xanthan and biapigenin derivatives. This plant has antioxidant and anti-inflammatory effects in intestinal or liver ischemia disorders.
