مطالعه فراوانی شگیلا توکسین -1 در جدایه های شگیلا فلکسنری از بیماران مبتلا به اسهال به روش Real-Time PCR
مطالعه ی فراوانی شیگاتوکسین-1 در جدایه های شیگلا فلکسنری جداشده از بیماران مبتلا به اسهال در شهراهواز به روش Real-Time PCR

زمینه و هدف: اسهال دومین عامل مرگ و میر کودکان زیر پنج سال در سراسر جهان می باشد. شیگلا یک پاتوژن بسیار بیماری زا است، که منجر به اسهال خونی به ویژه در کودکان می شود. شیگاتوکسین<span dir="LTR">Stx) </span>)<a href="#_ftn1" name="_ftnref1" title=""><span dir="LTR">[1]</span></a> یکی از قوی ترین سموم باکتریایی شناخته شده است، که مسئول شدید ترین تظاهرات شیگلوز از جمله اسهال خونی، کولیت هموراژیک و سندرم اورمی همولیتیک و<em> ... </em>می باشد<em>.</em><br>
با داشتن دانش کافی در مورد حضور این توکسین در این جدایه ها می توان درمان موثر و هدفمندی را پیش رو گرفت و از بروز عوارض بعدی، جلوگیری کرد. در این مطالعه فراوانی <em><span dir="LTR">stx1</span></em> در جدایه های شیگلا (به جز شیگلا دیسانتری) جدا شده از بیماران مبتلا به اسهال در اهواز مورد بررسی قرار گرفته است<em>. </em><br>
مواد وروش ها: تعداد 227 نمونه ی باکتری شیگلا ، شامل 157 جدایه ی شیگلا سونئی، 60 جدایه ی شیگلا فلکسنری و 10 جدایه ی شیگلا بوئیدی، از بیماران مبتلا به شیگلوز مراجعه کننده به بیمارستان های شهر اهواز که طی سال 1392 تا 1394جدا گردیده اند، جمع آوری شد. آزمون <span dir="LTR">PCR</span> جهت بررسی فراوانی ژن <em><span dir="LTR">stx1</span></em> انجام شد.<br>
یافته ها: نتایج حاصل از آزمون <span dir="LTR">PCR</span> برای این جدایه ها منفی بود و همه ی 227 جدایه ی شیگلا مورد مطالعه فاقد ژن<em><span dir="LTR">stx1</span></em> بودند.<br>
نتیجه گیری: فقدان ژن <em><span dir="LTR">stx1</span></em> در جدایه های شیگلا جدا شده از شهر اهواز، نشان دهنده ی این است که فاژ کدکننده ی شیگاتوکسین توسط این جدایه ها حمل نمی شود. اما با توجه به اینکه این فاژ در مناطق مختلف جهان و به طور مستقل ظهور پیدا کرده است و هم چنین شرایط انتقال به سایر گونه ها هنوز به طور کامل کشف نشده است، نمی توان به طور یقین حضور آن را در این مناطق منفی گزارش کرد و پیدایش احتمالی جدایه های حامل آن را در آینده، نادیده گرفت.

Diarrheal is the second most prevalent causes in under five-year old children worldwide. <em>Shigella</em> spp. which renders dysentery in children are mostly prevailed in underdeveloped countries, where hygiene were poor practices. Shiga toxin is responsible for fatal manifestations of shigellosis including dysentery, hemorrhagic colitis, uremic-hemolytic syndrome, etc. By increasing our knowledge on toxin presence in isolates, effective treatment can be done in future and prevented from further complications. we investigated the frequency of <em>stx1</em> gene in <em>Shigella</em> spp. (except <em>S. dysenteriae</em>) isolated from diarrheal patients in Ahvaz city, southwestern Iran.<br>
<strong>Methods</strong>: During a two-year period, 227 <em>Shigella</em> species including 60 <em>S</em>. <em>flexneri,</em> <em>157 S. sonnei</em> and 10 <em>S. boydii</em> were obtained from diarrheal patients. Upon DNA extraction, PCR was applied for <em>stx1</em> gene detection in validated isolates.<br>
<strong>Findings</strong>: Based on our findings, none of the organisms were recognized to be positive regarding the <em>stx1</em> gene.<br>
<strong>Conclusion</strong>: The lack of toxin production among the <em>Shigella</em> spp in this area shows that the encoding shiga-toxin phage is not carried by these isolates. However, regarding the emergence of this phage in other territories of the world and the unknown condition for transmission to other species, we can’t make an absolute decision on its absence in this region<span dir="RTL">.</span>
